Fill in the online form as shown below:

1. Select the amount you would like to donate.
2. From the dropdown box, select “Area of greatest need” as the area to which the donation is to be applied.
3. In the Dedication box, check the box indicating that the donation is in honour of someone and select “In Honour”, and in the box below, enter “Al and Millie De Souza Endowment Fund”.
4. In the Notification box, enter the name and e-mail address shown below.
5. Complete the billing information and submit the form. Your e-receipt will be sent within a few minutes.
